Culture Guide Acknowledgement and Waiver Form

ACKNOWLEDEMENT
I have received the link and reviewed the GREAT Culture Guide. The GREAT Culture Guide is a resource intended to serve as the handbook for all team members (volunteers, employees, and contract artists) that describes important information about GREAT Theatre.

I understand that I should consult my supervisor regarding any questions I may have regarding its contents. I understand and agree that nothing in the Culture Guide creates, or is intended to create, a promise or representation of continued contracts with GREAT. I have received the culture guide, and I understand that it is my responsibility to read and comply with the policies contained in this handbook and any revisions made to it.

WAIVER
As a contractor I agree to follow all safety guidelines and policies put forward by GREAT Theatre. I agree to release, indemnify, and hold harmless GREAT Theatre as well as all GREAT Theatre employees, agents, representatives, successors, etc. from all losses, claims, theft, demands, liabilities, causes of action, or expenses, known or unknown, arising out of my participation.
